Health, income and protecting your family

Health insurance and income protection in France: companies, individuals, self-employed

Top-up health insurance (mutuelle) and income protection (prévoyance) meet two distinct needs: completing reimbursement of your healthcare costs and protecting your income and family against life’s events. Whether you are an employee, an individual, self-employed or an employer, the contracts to review are not the same.

Understanding top-up health cover and income protection

Top-up health insurance pays all or part of the medical costs left after reimbursement by French state health insurance (Assurance Maladie), depending on the contract. Income protection safeguards your income and your family in the event of sick leave, disability or death. Individual health and income protection lets you review both needs personally.

Since the generalisation under the national inter-professional agreement (ANI), private-sector employers in France must offer group health cover to their employees, subject to the exemptions provided for. Companies can also set up group income protection, sometimes required by the collective agreement. Our group health and income protection page covers this at employer level.

Self-employed workers (tradespeople, shopkeepers, liberal professions, majority managers) have more limited basic protection, particularly for sick leave. Health cover for the self-employed (TNS) is reviewed according to the activity, the scheme and the family situation. The French “Madelin” tax framework may, under conditions, allow certain contributions to be deducted: it must be checked against the contract and current rules.

Personal protection goes beyond health. Mortgage borrower insurance covers loan repayment in the event of death or incapacity, while savings, life insurance and retirement plans serve long-term goals. The aim is to coordinate health, income protection and savings without overlap.

Frequently asked questions

What is the difference between health insurance and income protection?

Top-up health insurance completes reimbursement of medical costs. Income protection safeguards your income or family in the event of incapacity, disability or death.

Is company health cover compulsory in France?

Yes: private-sector employers must offer group health cover to their employees since the generalisation under the ANI. Some employees may be exempted in the cases provided for by law.

Can I take out individual health insurance?

Yes. An individual contract is reviewed according to your health needs, your family situation and any cover you already have.

What cover for a self-employed worker?

A self-employed worker can take out top-up health cover and income protection (daily allowances, disability, death) suited to their activity and income.

Does the Madelin scheme still exist?

For the health and income protection of the self-employed, the so-called Madelin tax framework still allows, under conditions, certain contributions to be deducted. The exact conditions depend on your contract and current rules.
